Improve DTMF handling in ast_read() even more in response to a discussion on
the asterisk-dev mailing list.  I changed the enforced minimum length of a
digit from 100ms to 80ms.  Furthermore, I made it now enforce a gap of 45ms in
between digits.  These values are not configurable in a configuration file
right now, but they can be easily changed near the top of main/channel.c.

Make the difference clear about what the responsibilities of the core and a spy are when it comes to spying on a channel. The core is responsible for adding a spy to a channel, feeding frames into the spy, removing the spy from the channel, and notifying the spy that is has been detached. The spy is responsible for reading frames in, and cleaning itself up. Each side will not try to do the other's job.

This update fulfils the request of bug 7109, which claimed the language arg to ast_stream_and_wait() was redundant. Almost all calls just used chan->language, and seeing how chan is the first argument, this certainly seems redundant. A change of language could just as easily be done by simply changing the channel language before calling.

Convert the PBX core to use read/write locks. This yields a nifty performance improvement when it comes to simultaneous calls going through the dialplan. Using murf's test the old mutex based core took an average of 57.3 seconds while the rwlock based core took 31.1 seconds. That's a nifty 26.2 seconds performance improvement. The other good part is that if we do need to switch back then we just have to change the lock/unlock API calls. I converted everywhere that used to touch the mutex locks directly to use them.
